  This position will work in the department of Lifestyle Medicine. Lifestyle Medicine has two locations in the Magic Valley. This position will primarily work in the North College location serving the Pulmonary Rehab and POTS Rehab Clinic.

    • We use a case management model and each clinician is responsible for care coordination of their assigned class/group of patients during their rehabilitation course.
      
    • Our team is multidisciplinary including the following disciplines: Medical Director, Registered Nurses, Exercise Physiologists, Registered Dietitian, Respiratory Therapist, Physical Therapist, Social Workers, and Patient Access Specialists, to support the needs of our patients.
