St. Luke's home health team of skilled professionals provides
comprehensive health care in the warmth and comfort of the
patient's home. From newborn to seniors, we offer the latest home
health techniques and technology to people at all ages and stages
of life. Dedicated to the continuum of care as patients transition
from inpatient to home to outpatient settings for care. Our
home-based services team (NP, RN, MA, MSW) also assists with
oversighting transitions. Close working relationship with St.
Luke's Hospice team. You will provide comprehensive care to
patients with a new diagnosis, medications, or have been discharged
from ER, require respiratory treatment, and wound care. During home
visits, you will ensure patients are progressing with their care
plan, provide education related to their disease process, and
assess physical and psychological needs to improve quality of life.
When it comes time to discharge, you will coordinate resources and
educate patient and patient's family. This is a direct care
position providing in home care that requires traveling within the
Magic Valley area. What you can expect from this role
- Part Time - three days a week (8:30a-5:00p) with rotating
evening/overnight call and weekends
- Provide individualized care for patients from children to
elderly
- Manageable case-loads (Daily visit requirements below state and
national average)
- Pay for drive time
- Mileage reimbursement (Geographical Area: Twin Falls, ID and
it's surrounding areas)

- Education: ASN or BSN Degree
- Experience: 1 year of RN experience; Home Health or Acute Care
experience helpful
